In-network direct provider with Blue Cross Blue Shield, Cigna, Aetna and Johns Hopkins (EHP)

I will handle all billing.  I will bill for other insurances as well if acupuncture coverage is provided.  For those with insurance coverage, only a co-pay is due at the time of treatment.  For those without insurance, payment is due at the time of treatment.
